7th and 8th Grade Holmes Athletics
All student athletes must complete a new online registration each year, including uploading a current signed physical form by a physician.
PLEASE DO NOT TURN IN ANY ATHLETIC PAPERWORK INTO YOUR COACH.
We are excited to announce that the Holmes Athletic Department has partnered
with FinalForms, an online forms and data management service. FinalForms allows
you to complete and sign athletic participation forms for your students. You may
review your data at any time. You will be required to sign your forms once per year
and after any update.

your account, create your students, and sign your forms.

PLEASE NOTE: STUDENTS CANNOT PARTICIPATE IN PRACTICES OR GAMES UNTIL
THE ATHLETIC PACKET IS COMPLETED BY THE FIRST DAY OF PRACTICE.
